John Fawkner Private Hospital are seeking a passionate and creative individual to join their small and supportive team as a Discharge Planner on a permanent part-time basis.

 

About us:

John Fawkner Private Hospital is located in the northwest of Melbourne.  With 186 registered beds, a 12 cubicle Emergency Department, 4 operating theatres, 12 bed critical care complex and busy Day Procedure Unit the hospital provides a wide range of services to its community.  The hospital specialises in orthopaedics, general surgery, general medicine, oncology, cardiology, urology, plastic surgery, ENT and gynaecology.

 

As Discharge Planner your responsibilities will include:

·       Being responsible for providing optimal discharge planning in line with the strategic direction, operational processes, budget and business direction of the hospital

·       Delivering seamless patient flow, effective bed utilisation and episode management as part of a dynamic team

·       Liaises with Unit Managers, nursing staff, allied health Professionals and Medical Practitioners to coordinate patient discharge plans

·       Liaise with external service providers for patients with ongoing care needs post discharge

·       Supports and facilitates the patient and family by providing appropriate information and options regarding health and nursing care

·       Early screening of inpatients with complex care needs to facilitate timely discharge

·       Ensures information relating to patient discharge plans is documented and accessible by all clinical staff

 

Selection Criteria:

·       Between 2 to 4 years as a Registered Nurse working in an acute setting

·       Good Interpersonal and time management skills with an ability to work within a multidisciplinary team

·       Able to generate referrals to relevant organisations

·       Ability to support and liaise with respective NUM’s and Departments

·       Ability to problem solve in a fast-paced environment

·       Ability to work autonomously and within a team

If you are successful, you will receive a salary based on your years of relevant nursing experience against the current nursing Enterprise Agreement the "Healthscope - Victoria - Nurses' and Midwives' Agreement 2021-2025" (salaries range between $35.33 and $49.25 per hour + Superannuation).
